Position Summary: Bellefaire's Street Outreach Program is looking to add a Street Outreach Worker to the team. Staff are needed to go in the field and assess homeless youth's immediate needs as well as will need to be able to engage at-risk teens and their families.

Responsibilities Include:

• Perform community outreach and client assessment by preparing individual service plans under service teams
• Provide case management, service coordination, and ongoing aftercare services
• Conduct data collection and case documentation
• Represent the agency in the maintenance of relationships with other organizations in relation with the client and their families

Qualifications:

• Minimum Bachelor's Degree in Social Work or related field
• Must own car, cell phone and perform job responsibilities with a high degree of initiative and independent judgment.
• Experience working with youth by providing crisis intervention, service coordination, skill building and family education a plus.
• Sensitivity in relating to persons of varying backgrounds and demonstrate ability to work with diverse groups of people possessing various strengths, aptitudes and abilities.
