SUMMARY:

The Basic Center Coordinator is responsible for implementation and ongoing operations of the Basic Center Program. This position provides direction, coordination, and clinical and administrative cordination to the Basic Center staff.  Additionally, the Basic Center Coordinator monitors the host home program, including utilization of host home bed days, as well as works to promote development of the program in the community. This position requires varying shifts, including evenings and weekends. Services will take place at various places in the community throughout Cuyahoga County. Outreach on the street is a requirement of this position as well as being available by phone and in person if needed to host home family members. The Basic Center Coordinator will ensure that all staff are firmly committed to utilizing the Positive Youth Development approach. 

ESSENTIAL DUTIES:
 
  • Provide clinical coordination and leadership to Basic Center Program and its staff.
  • Coordinate and perform client assessment, including but not limited to, Psychiatric Diagnostic Evaluations, Individual Treatment Plans, and other program client assessments.
  • Work collaboratively with the Agency’s Foster Care Department to identify and train foster families to be eligible host home placements.
  • Ensure effective utilization of host home placements, including but not limited to, providing support during placement, monitoring the use of bed days according to grant requirements, preparing for family reunification, and linking to other services as needed.
  • Coordinate and conduct data collection and case documentation, including the submission of data for required 6 month program reports.
  • Ensure that all grant requirements are met, recorded, and submitted to UMTYS department leadership throughout the entire grant cycle.
  • Work collaboratively with the Street Outreach Specialist and team to ensure continuity of care for homeless youth.
  • Answer RHY hotline phone during typical business hours and other times as assigned.
  • Represent the Agency in the development or maintenance of relationships with other organizations, including community agencies, public agencies, and referral sources.
  • Maintain high standards of ethical and professional conduct.
  • Possess, employ, and train staff in, a practical understanding of:
    • basic child, adolescent, and family development.
    • issues that are relevant to homeless youth.
    • interventions used in helping runaway and homeless youth.
  • Contribute to the development and maintenance of the record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
  • Ability to qualify as an approved Agency driver.
  • Own a cell phone to be used for on-call responsibilities.
  • Attend scheduled staff meetings, supervision, and on-going training that will provide the skills necessary to implement the program.
  • All required trainings, certifications, and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  • Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  • Assure that program staff maintain high standards of ethical and professional conduct.
  • Contact Director of Operations- RHY with any emergency situations (i.e. in need of clinical support, AWOL, safety concern for yourself or the well-being of a client or family member, a client threatening to harm themselves or others).
  • Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  • Other duties as assigned by management.

QUALIFICATIONS:
 
  • Education: Minimum Master’s degree in Social Work or related field.
  • Licensure: Prefer valid Ohio LSW, LPC, or minimally two years’ professional experience working with youth and families.
  • Skills: Experience working with youth by providing crisis intervention, service coordination, skill building, and family education. Demonstrated cultural competency and sensitivity in relating to persons of varying backgrounds and demonstrated ability to work with diverse groups of people possessing various strengths, aptitudes, and abilities. Ability to perform job responsibilities with a high degree of initiative and independent judgment. Demonstrated oral and written communication skills and effective interpersonal skills. Proficiency in using Agency computer systems and software as required to perform essential job functions. Practical and creative problem solving skills.
  • Experience: Proven substantial experience serving Runaway and Homeless Youth, including knowledge of addressing issues of alcohol and other drug use. Proven experience working with troubled teens and their families  Practical and/or clinical understanding of the underlying issues that lead to youth homelessness.
